If you possess private health insurance we will contact your health fund prior to your procedure to ensure that you are eligible for surgery under your level of cover.
We will provide you with an Informed Financial Consent which will outline any potential costs associated with the hospital portion of your procedure.
We also recommend contacting your health fund directly with any questions you have regarding your health cover.
Please note you may incur other costs related to your procedure that are not covered by your health fund, these can include but are not limited to the following:
- Surgeon fees
- Anaesthetist fees
- Pharmacy (medication required during or after your admission)
- Pathology (blood tests, skin samples, polyps, etc)
- Imaging or X-Ray
- Medical and Allied Health Practitioner fees
If you are not covered by private health, or any other form of cover, you will be required to cover the cost of your procedure.
We will issue you with a quote for potential costs for the hospital portion of your procedure. This quote is valid for 30 days from the date of quotation.

If you are covered for your procedure through ADF, we require the following information from you:
- Defence Approval Number (DAN)
- EPID/PMKeys
If you are a DVA card holder, please provide us with your DVA card number.
- Gold Card patients are covered for all care
- White Card patients may be covered but are subject for approval by DVA
Prior to your admission, you or your organisation are required to complete a Workers Compensation Approval Form.
A credit card numberĀ or an approved insurance policy and claim number are required before the procedure can commence.
If you are insured with any overseas company, you will be required to either pay the estimated cost on admission or provide us with a claim approval prior to your procedure.
